The goal of integrating Human Performance into the incident investigation process is to facilitate the “modern view of incident causation”. This means that human error or non-compliance that contributed to incidents are not seen as a root cause but rather as a symptom of issues deeper in an organisation. In other words, there is a shift in focus from the individual behaviour to the underlying systemic and organisational factors.

Dr Marcin Nazaruk helps us to look at how this can actually work in practice using a dropped object incident as a case study.
